The Solstice DHMO Dental + Vision plan bundles comprehensive dental and vision coverage into a single plan with no deductible, no waiting periods, and no annual benefit maximum on the dental side. Coverage begins on your effective date, so you can schedule a cleaning or eye exam right away without waiting months for benefits to kick in. There are also no claim forms to fill out — your copay at the time of service is all you need to worry about. On the dental side, most preventive care — including routine cleanings, X-rays, oral evaluations, sealants, and fluoride treatments — is covered at $0 when you use a network general dentist. Basic and major services like fillings, crowns, root canals, extractions, dentures, implants, and orthodontics are all covered at fixed copays listed in the plan's Schedule of Benefits, so you know your out-of-pocket cost before treatment begins. If you need a specialist such as an oral surgeon, endodontist, or periodontist, you can go directly without a referral and receive a 25% discount off their usual fees, or get prior authorization to pay the lower listed copays. The vision benefit, powered by UnitedHealthcare, covers a comprehensive eye exam once every 12 months for a $10 copay. Eyeglasses (lenses and frames) or contact lenses are available once per year for a $25 copay, and there is a $100 frame allowance. Formulary contact lens fitting, evaluation, and follow-up visits are covered in full after the copay. If you prefer to see an out-of-network vision provider, the plan reimburses a portion of your costs. A few things to keep in mind: dental benefits apply specifically to the Solstice network of general dentists, and some copays marked in the Schedule of Benefits do not include lab or material fees, which can add to your out-of-pocket cost. Certain services like crowns and bridges may have additional lab charges depending on the material chosen.
