This plan pairs a PPO dental benefit with an annual vision benefit, giving you two important types of coverage under one enrollment. On the dental side, preventive services — including two cleanings per year, routine X-rays, oral exams, fluoride treatments (for children under 16), and sealants (for children under 16) — are covered at 100% both in-network and out-of-network, and the $50 individual annual deductible is waived for these preventive visits. Basic services like fillings, simple and surgical extractions, root canals, and non-surgical gum treatment are covered at 80% after the deductible, while major work such as crowns, bridges, and dentures is covered at 50%. The annual maximum benefit is $1,500 per person per calendar year. Orthodontic services are not covered under this plan. The vision benefit refreshes every 12 months and covers a comprehensive eye exam with just a $10 copay when you visit an in-network provider. If you need glasses, you pay a $25 copay for lenses and frames combined, and you receive a $100 allowance toward frames. If you prefer contact lenses, the same $25 copay applies, and formulary contacts — including fitting fees and up to two follow-up visits — are covered in full after your copay. If you choose disposable contacts, up to 4 boxes are included through an in-network provider. Children up to age 19 also receive polycarbonate lenses with standard scratch-resistant coating covered in full. If you visit an out-of-network eye care provider, you pay upfront and are reimbursed up to set allowances. The plan is offered through the Solstice network, which includes both private practice dentists and retail vision chains nationwide, giving you good flexibility in choosing providers. The dental annual maximum is $1,500 per person, so if you anticipate significant major dental work in a single year, you may have out-of-pocket costs beyond that cap. Orthodontic treatment is not covered. For major dental procedures expected to cost over $300, it is a good idea to request a pre-treatment estimate from your dentist so you know what your share will be.
